Upper molar intrusion technique with mini-implants: clinical case report

Abstract

Aim: to describe two clinical cases with the use of orthodontic mini-implant (OMI) for intrusion of upper molars to enable antagonistic prosthetic rehabilitation. Case report: case 1: female patient, 50 years old, with severe dental extrusion of teeth 16 and 17 with vestibularization of the lower incisors. The proposed treatment consisted of intrusion of the affected teeth through the installation of OMI in the buccal and palatal alveolar bone with chain ligature, with subsequent use of fixed lower orthodontic apparatus and finalization with prosthetic rehabilitation. Case 2: female patient, 36 years old, tooth 36 missing and 26 extruded with impairment of inferior rehabilitation. For this case, it was planned to install OMI in the alveolar bone only at the buccal bone, in a region between teeth 26 and 27, and another per palatal between teeth 25 and 26, together with the chain ligature covering the occlusal face of tooth 26. The intrusion proved to be effective after three months of treatment for both cases, demonstrating the correct use of forces. Final considerations: the intrusion of maxillary molars with the use of OMI is an effective and safe technique, which reduces the unwanted effects on adjacent teeth, and provides control of vertical tooth movement in a faster treatment.
